Tips for registration

• You are a Prescriber/Resident

• Illinois State License Number

  • Select the appropriate 3-digit prefix in the drop down. The 3 digit prefix corresponding to Temporary Physician Initial Licensure is 125.

  • Enter the remaining 6-digits of your Illinois State License Number in the appropriate box.

  • If you forgot your Illinois State License Number, visit the following lookup tool, under License Type select MEDICAL BOARD. You can then search by entering your First Name and Last Name. Other information is not required.

DEA Number

• For the ILPMP registration, the DEA number field should be 13 characters

• Enter the hospital’s DEA number followed by your unique DEA 4-digit code

• Hint: Do not enter any dashes or spaces

• Hint: The hospital’s DEA number is a 9-character code (2 uppercase letters followed by 7 numbers)

• Hint: For your unique code, only enter the 4-numbers. Do not enter the letters that precede them.
